The Document Management application houses various forms that students in the College of Education are required to complete throughout their degree program. The Document Management app is available within the GCU Portal.
Select the appropriate heading below for assistance with any of the forms available in the Document Management app.
If you’re required to complete this form, it will appear inside the Document Management app after you’ve posted in your course the first time.
The Clinical Field Experience Verification Form (CFEVF for short) is such a common issue that we’ve made separate guides just for it! If you’re required to complete this form, it will appear inside the Document Management app after you’ve posted in your course the first time.
- Access the Document Management App and select the form title. Then select Add New Entry.
- Complete the form and submit it. You are required to enter information about the facility, your mentor, and a brief description of any activities you completed.
NOTE If you completed hours at multiple facilities or with multiple mentors, simply add another entry and submit the additional form.Once the form is submitted, the status will change to Out for Signature and you will see the Birth to Pre-K evaluation in the document list as well. You do not need to fill out the evaluation form, as that is only for your mentor.
- Your mentor will receive a copy of the form via email. The email contains instructions for approving the hours you previously entered.
- You can check the status of each form inside the Document Management application by clicking the form title. Once a document status shows as “Complete”, it is ready to download. You should also receive an email notifying you that the form is available for download after your mentor completes it.
Additionally, if your form has expired (this occurs 4 years (1460 days) after the course start date), reach out to your Field Experience Counselor for guidance. Technical Support does not have paper copy versions of the ECS Birth-PreK form.
- Students in EAD-529 or EDA-586A will generate the Ed Admin Evaluation 529
- Students in EAD-539 or EDA-586B will generate the Ed Admin Evaluation 539
Both forms follow the same workflow, which starts after you post in your course the first time.
- First, your Faculty Supervisor will receive the form via email. They can click the link provided to fill out and sign the form.
- Once your Faculty Supervisor completes the form, it will be available for download in the Document Management App under the Instrument for Measuring Leadership Effectiveness section. If the document status shows as “Requirement Met”, then the process is complete, and the form is ready to download. You should also receive an email notifying you that the form is available for download after your faculty supervisor completes it.
NOTE You may receive an additional email asking you to “Approve” the form after your faculty supervisor completes it. Student approval is not necessary for the form, so you can skip this step.
- First, your Faculty Supervisor will receive the form via email. They can click the link provided to fill out and sign the form.
- Once your faculty supervisor completes the form, it will be available for download in the Document Management App under the Clinical Practice Evaluations area. If the document status shows as “Complete”, then the process is complete, and the form is ready to download. You should also receive an email notifying you that the form is available for download after your faculty supervisor completes it.
FAQ
After clicking on Remind, a dialog box will appear enabling you to write a brief message. When done, click on Send Reminder. The mentor will be sent a new link to the document.
If you have completed more hours than what is required for your class, the extra hours will appear in the Overages section. For example, if you completed 24 hours instead of the required 20, you’ll see 4 hours listed in the Overages section. If you have any questions about completing more hours than what is required or whether this applies to you, please contact your Field Experience Counselor.
Sometimes the classes that you take get updates and those updates change the required hours. When there are updates to the program and required hours, you will see the Equivalent Courses section. If you have any further questions about your program version update and required update, please reach out to your Field Experience Counselor.
When you fail, retake, or drop a class the recorded hours will not count towards the hours for the program. You will see the classes that are not counted towards the hours in the Assignments Not Counted section. Additionally, courses with recorded hours from a previous program but are not required in your current program will also be listed here. While you will need to start these classes from scratch for them to count, you can still access previous entries by clicking on the course.
Mentors should receive an email with a link to whichever form they are responsible for signing. If a mentor is unable to open the form on their work computer, they can try to access the form from a mobile device (smartphone or tablet) or forward the link to a personal computer.
Some forms are completed by faculty supervisors who have a GCU email address. If this situation applies to you, your faculty supervisor can also access the form they need to sign by navigating to https://documentsign.gcu.edu/. To access the website, faculty supervisors should log in using their @my.gcu.edu or @gcu.edu email address and password.
