As a student of the College of Nursing and Health Care Professions, you may have practicum hours to complete as part of your degree program. If so, you will be required to fill out a Field Experience Site Information Form. The form can be accessed by logging into your GCU Portal and selecting the eDocs app. Click the link: "Click here to access all available documents pending e-signature". Select the Field Experience Site Information Form from the list of available eDocs. NOTE If the form is not available or you see a message that says, "0 documents pending e-signature", contact your Field Experience Counselor (FEC) and they can add the form to your portal. The form will ask you to enter information about your prospective practicum site and mentor. After the form has been filled out and submitted, it will be sent to your Field Experience Counselor. You will also receive a copy of the completed form in your GCU email. Save your copy, as you will be required to upload it to your classroom when you start your practicum course. FAQs Expand/Collapse All What is a preceptor? The Field Experience Site Information Form asks you to enter information about your preceptor/mentor. This term is used to describe the mentor who is supervising you at your site. Why doesn't the form open for me? If you see the form available in your eDocs app, but it doesn't open, disable your browser's pop up blocker. The form will time out after an extended period of time. If you know that your responses to the short answer questions are going to be extensive, we recommend typing your responses in a word document. Once all of the questions have been answered, you can copy and paste your responses into the form. Can I fill out the form using my phone? We recommend using a full desktop or laptop computer to fill out the form. When I try to enter a city, I receive an "Invalid City" error. What should I do? There is a known issue with the "City" field in this form. It will not accept cities that contain two or more words. As a temporary workaround, enter the city without spaces. For example, type: "LasVegas" instead of "Las Vegas". We are working on getting this fixed, so thank you for your patience in the meantime.
